The crack created both a sharp edge and a pinch point.

    During the course of a complaint investigation, Staff Person #1 admitted to taking Child #1 into a facility bathroom while the child was having behavioral issues. While in the bathroom, Staff Person #1 admitted to yelling loudly at the child, threatening them with physical harm. Two other enrolled children were present in the classroom area when this occurred.

    During the assessment of supervision and ratio at 9:30 AM on 1/5/22 in the Infant Room, Staff #1 named 1 Infant and Staff #2 named 2 Infants and 2 YT.
