These tears prevent proper sanitation. Cert rep observed the stacked boxes and toy fabric blocks with tears were removed from the facility. During an announced monitoring inspection on 1/21/26 Cert rep observed the following incident related to supervision of children: In the afternoon, in the Red Room, there was 16 children observed to be present with Staff 1 and 2. Staff 1 named and identified 7 children as being in their assigned group. Staff 2 named and identified 8 children in their assigned group. When Cert rep questioned this, it was determined that 1 of the children present was named and identified by both Staff 1 and 2 as being in both of their groups. 2 children present were not named by either staff and were not assigned to either staff.
