While at the facility unannounced on 8/7/24 the inspector observed the outdoor play space and observed a concrete pad with a built-in sand box. This sand box did not have any sand in it and created a hazard for the children running and playing outside.

A renewal inspection occurred at the facility. A physical site review was conducted. Repairs were made to the outdoor play area fencing. The fencing was able to be lifted enough off the ground that children could crawl under. Cut metal stakes were added to hold the fencing from being lifted up. These stakes were sharp and exposed in the play area posing a hazard to children playing in the area.

The outdoor metal climber contained peeling paint, rusty metal, broken plastic protector on the chain climber and the surface covering on the flat parts was coming off. These areas were addressed with maintenance who visited during this inspection and with staff.
