During a complaint investigation on 7/17/25, certification rep reviewed footage of an incident that occurred on 3/3/25. Staff # 1 was observed leaning over a child and rubbing their back during nap time. Two children were behind staff # 1 that were not properly supervised; another child was to the left of staff # 1. During the 35 seconds of footage reviewed, staff # 1 did not look at the child to the left. Staff # 1 was terminated after this incident.

Staff # 1 extended their leg and nudged child #1 on their chest two times. This was done to get child # 1 to stop making noise during nap time. Staff # 1 was terminated immediately for the incident.
