This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all staff to receive a minimum of three hours of training regarding Supervision. The training must be PQAS approved, in-person, and outside of childcare hours. The legal entity must receive DHS approval of the training content prior to scheduling the training. The operator shall provide a correction date for when the training and follow up with Quality Coach will be completed. 3. The facility will demonstrate compliance with 3270.113(a) and 3270.113(a)(1) for a period of three months. There may be no instances of noncompliance during this time period during unannounced inspections. The operator shall provide a correction date that is 3 months from the date of plan submission.
A complaint inspection was conducted 6/8/26. During the investigation, it was discovered that on 5/28/26 Child #2 was left on the playground. Video footage showed that Child #2 was left on the playground from 11:53-11:57am with another staff and group of children before being brought back to their classroom by another staff. Child #2 was in the primary care group of Staff Person #2. Staff Person #2 was terminated from employment at the facility on 6/1/26. Through staff interviews, it was discovered, that a lapse of supervision occurred on 6/19/26. Video footage showed Child #2 and Child #3, both young toddlers, attempting to open the gate to the playground fencing and eventually succeeding. The children then began walking down the sidewalk outside the playground. This incident was unnoticed by Staff Person #3. The children were outside of the playground fence for less than a minute before being discovered.

A complaint inspection was conducted 6/8/26. During the investigation, video footage of Room 17 was reviewed from 6/4/26 at 2:47pm. At that time, Child #1 is seen banging a book on a shelf. Staff Person #1 leans over the shelf and roughly jerks the child up and then back down to the floor before blocking the view of the camera. Staff interviews discovered that while blocking the camera, Staff Person #1 had pushed the child down again, grabbing the child by the wrist and then scolding the child, Staff Person #1 is heard in the video saying "All done, no thank you. Your friend is sleeping. You are not being nice. No" Child #1 crawls away toward another shelf. Staff Person #1 walks toward Child #1 and leaning over the next shelf and pushes the child roughly by their head. Staff Person #1 was terminated from employment at the facility on 6/5/26.

A complaint inspection was conducted on 3/23/26. During the inspection, it was determined through staff interviews that a lapse of supervision occurred on 3/2/26 when Staff Person #1 was diapering Child #1 in the bathroom area of the classroom. Staff Person #1 could not see the children in the classroom. During this time, Child #2 left their cot and pulled down the pants of Child #3.

During the inspection, it was determined through staff interviews that Preschool Room 4 room is frequently out of ratio during naptime from 12:30-2:30 when approximately 3 out of 18 preschool children nap and many children are up and off their cots. During naptime on 3/2/26, Staff Person #1 was alone with 18 preschool children during naptime but many children were up and off of their cots during that time.
