Through interviews, it was found that the following has occurred at the child care center: Staff Person #1 spanked their own child, Child #2, while saying "I told you I'd hit you if you didn't stop." This occurred at the child care center while the child was in care. Staff Person #1 has yelled aggressively at the children, "Go eat your food" "I'm gonna beat your ass." "I'm not going to deal with your cranky shit today." Staff Person #1 and Staff Person #2 have been seen grabbing children by the biceps, slamming them down into chairs. Staff Person #2 was heard telling a child "I'm going to pull your hair, I'm not playing with you," followed by the child yelling "ow." Staff Person #1 and Staff Person #2 were terminated from employment on 9/18/25.

Staff Person #1 was terminated from employment on 9/18/25.

At that time, exposed bolts were observed on the fencing of the school age playground, the young school age playground, the preschool playground, and the toddler playground. A basketball hoop was observed on the young school age playground. The weighted portion of the basketball hoop contained green, standing water. The spaceship climber on the toddler playground was observed to be cracked on the slide posing a pinch hazard. Small debris was trapped underneath the mats on the toddler playground where the play equipment was located. The debris caused the mats to be uneven, posing a tripping hazard to the toddlers.
