The embedded equipment with the slide on the toddler playground shall be made inaccessible and will not be used by the children until such time as the documentation showing that the playground surfacing meets the recommendations of the United States Consumer Product Safety Commission is on file at the facility. For this piece of equipment, the documentation must show that the playground surfacing is safe for a fall height of at least 2 feet 2 inches. This portion of the plan shall have an immediate correction date. 2. The operator will obtain the documentation showing that the playground surfacing meets the recommendations of the United States Consumer Product Safety Commission and keep this documentation on file at the facility. For this piece of equipment, the documentation must show that the playground surfacing is safe for a fall height of at least 2 feet 2 inches. The operator shall provide a correction date for when the documentation will be obtained and placed on file at the facility.

    A renewal inspection was conducted 7/9/25. At that time, embedded playground equipment in pour-in-place surfacing was observed on the toddler playground. The documentation from the installer providing the critical fall height could not be found during the inspection.

    Child # 1, #2 & #3 were outside on the facility play space unsupervised with a member from another agency Amaing Athletes. Two facility staff were outside with 6 Pre-K aged children. When asked by certification rep, the 2 staff outside said they were not responsible for child # 1, #2 or # 3, who were with the Amazing Athletes person. Rep was told by the 2 staff members, that the staff responsible for child # 1,2 & 3 was inside the facility with the rest of their group. The facility director provided certification rep with a statement from Amazing Athletes that states: "The children they provide services to will remain under the care, direction and supervision of the school while receiving instruction from Amazing Athletes and Super Soccer Stars."
