An unannounced inspection was conducted 4/29/26 as a result of self-reported noncompliance. During the inspection, it was discovered that a lapse of supervision occurred on 4/28/26 at approximately 12:00pm. Child #1 left the Intermediate 1 classroom through the door to the fenced-in playground. Child #1 was in the primary care group of Staff Person #1. Neither Staff Person #1 nor Staff Person #2, who was also in the Intermediate 1 Room, were aware that Child #1 had left the classroom. Child #1 was on the playground for less than one minute before being discovered by Staff Person #3 and brought back to the Intermediate 1 Room.

A complaint inspection was conducted on 1/6/26. At that time, the temperature in the Pre K1 and Pre K2A classrooms was below 65° F. The temperature was 60° F in PreK 2A and was 59° F in PreK 1. The facility reported that the heat had not been working in the classrooms since 12/15/25 and they were waiting on parts to have it repaired. The classrooms were not in use while the temperature was below 65° F.

Staff person #2 was also in the room prior to Staff Person # 1 leaving the room and left at the same time as Staff Person # 1 forgetting that there was still a child in the room. An acceptable plan of correction was received on 8/14/2023 with an implementation date of 8/28/23. On 9/8/23 child #1 walked from the Beginners classroom through the adjoining bathroom to the connecting Intermediate classroom next door, which was unoccupied. Staff#1 was directly responsible for the supervision of this child, in addition staff person #2 and Staff Person #3 were also present in the room and all were unaware that the child left the room until the Assistant Director brought him back approximately 3 minutes later. An acceptable plan of correction was received on 8/14/2023 with an implementation date of 8/28/23. On 10/11/23 Staff Member #4 was inside a classroom with 4 children. She left the classroom to return to the outdoor play space and took 3 of the 4 children outside with her, leaving child #4 alone inside the classroom for approximately 1 minute. Staff Person #5 found child alone in the classroom and returned her to the group.
