A complaint inspection was conducted 6/26/26. Through the investigation, it was discovered that Child #1 was injured at the facility on 4/21/26 when their fingers were pinched in the hinged side of the door to the playground when a parent opened the door from inside the building to enter the playground.

An unannounced inspection was conducted on 12/20/21 at facility, while on site in the 2 year old classroom, certification rep observed child # 1 restrained in a feeding moon shaped table by themself with no food or snacks. Child # 1 was separated from the other children and staff in the classroom. Certification rep asked facility person # 1 why child # 1 was restrained in the feeding table chair and it was not snack or lunch time. Facility person # 1 stated "they are a biter".
