The large, plastic, ceiling light cover was not attached on all four corners, as it was hanging down from the ceiling and could create a falling hazard. In the outdoor space, two pieces of green outdoor carpet were ripped and peeling apart, which could pose a tripping hazard. The first one was located under two, plastic picnic tables, and the second one was located under the mini slide equipment and plastic playhouse.
