When certification representative arrived at facility to conduct an allocated unannounced monitoring on 5/29/2025, there were two staff members outside with preschool aged children. Certification representative conducted a supervision/ratio check and was told that there were 22 preschool-aged children in the group. Staff Person #4 alerted another staff person to have Staff Person #3 come out as soon as possible. Staff Person #4 identified 10 children as being in their group, Staff Person #2 identified 10 children as being in their group, and Staff Person #3 identified 2 children as being in their group. Certification representative conducted a headcount and there were 23 total preschool-aged children in the group. At that time, staff realized that one of the children's cards was stuck to the back of others and reported that they had just come back from therapy. Additionally, certification representative conducted supervision/ratio check in the Preschool Room and was told that there were 13 preschool-aged children in the room. Staff Person #1 identified 6 children as being in their group and Staff Person #6 identified 4 children as being in their group. Certification representative advised that the two groups total 10, which was not the reported number of children in the room. Staff then made several attempts to identify their groups of children. Staff referenced the chart on the wall with the groups listed. Staff Person #1 then identified 7 children as being in their group and Staff Person #6 identified 5 children as being in their group. Certification representative conducted a headcount, and it was determined that there were 12 preschool-aged children in the room. Staff reported that there was a new child in the room and another child went home early today.
