The legal entity will confirm continuing compliance by sending copies of the documentation to the certification representative on a weekly basis for a period of three months after DHS approves the documentation format. 3. The Legal Entity must arrange for all facility staff to receive a minimum of two hours of training regarding maintaining staff: child ratios. The certification representative will make the training referral to the appropriate ELRC and approve the content of the training. Documentation showing all staff completed the required training will be sent to certification rep by the trainer.

    At time of renewal inspection on 3/4/26, two classrooms were observed to be out of ratio during the facility walkthrough. The Pre-K classroom was observed to have 12 pre-school aged children with staff person #6, prior to the assistant director going in the classroom. The Infant B classroom was observed to have 6 young toddlers with staff person #5.
