Staff #1 completed Mandated Reporter training on 9/22/24, which was not completed within 90 days of hire. Staff #2 was missing an out of state child abuse clearance. Clearance has been applied for but is not completed and in the staff's file. Staff #2 may not work in a child care position until the clearance is obtained.

The file of facility person #2, hired 9/14/21, contained a Dept. of Education FBI clearance dated 8/23/21, however did not contain a completed DHS FBI clearance as required under the CPSL. The file of facility person #3, hired 11/22/21, did not contain a completed FBI clearance or NSOR certificate. There were copies of requests for clearances on file, however the request date for the FBI clearance was 11/29/21. Additionally, the request date for the state police and child abuse clearances were 11/23/21. The facility does have an approved provisional hire waiver dated 1/28/2020, however the requirement to have copies of the requests for all clearances on file at the time of hire was not met.
