On June 27th, 2024, the Director Self-Reported loss of supervision that occurred June 26th, 2024, at 4:42pm. Child#1 was left unsupervised on the facilities fenced playground for 15 minutes. The time was verified by camera footage. Staff members #1, #2, and #3 were on the playground in charge of the children, and none had the child in their primary care group. A YMCA member noticed the child on the playground with no staff present and carried the child into the facility. Staff did not realize child was left behind.
