Staff #2 identified 13 School age children in their group which put her out of ratio. When given another opportunity to identify their groups again they refused. Upon reviewing the emergency contact forms it was determined that there were 12 preschoolers and 10 school age children again putting them out of ratio. Upon returning to the facility, it was observed that there were toddlers inside even though the staff #3 that answered the door both times (9:25am and 10am) had said there were no children in the facility when she originally answered the door. Staff #3 left the children alone in the classroom to answer the door both times. It was observed that Staff #3's granddaughter an older school age child was in the classroom with the 6 toddler age children. When asked Staff #3 named the 1 young toddler and 5 older toddlers but did not name the 1 older school age child. Staff #3 was out of ratio. Upon review of emergency contact forms, it was determined that there were 4 young toddlers, 2 older toddlers, and 1 school age child which still left Staff #3 out of ratio.
