At the bottom of the swings, slide, ladder the mulch had been kicked/washed away and dirt was exposed. The play area is on a hill and the rubber mulch was gone. At least 9" of loose-fill wooden mulch (or 6" of loose-fill rubber mulch) is required around all embedded equipment. These areas were identified with the Director at the time of the inspection.

Staff #1 was also observed picking Child #1 up by the arms and legs and dropping the child to the ground. Staff #1 was also observed restraining, using their leg, Child #1 to their nap matt. The Child #1 is viewed on several occasions, without success, attempting to get away from Staff #1 and out from under the leg of Staff #1.

During unannounced complaint investigation on 2/9/23, I checked the sink vanity cabinet in the baby room. The cabinet had doors on it, but the doors did not have any type of lock on them.
