Through interviews with facility staff and the training institute, it was determined that the trainer did not have the facility complete a sign-in sheet for proper identification and verification purposes at the time of the firesafety training conducted on 10/2/25. The trainer relied on the facility to provide them with a list of attendees. The list provided by the facility to the certification representative also did not contain a signature from the trainer verifying the attendance, and as such, is not accepted, as it does not verify the staff who were present during the training.
