3270.113(a)(1) · Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.
During the inspection of the Preschool 2 childcare space [observed in the outdoor play space], staff person #1 and #2 only identified 7 children out of the 10 children present at that time. The reason that was given was that the two staff persons had not correctly adjusted the children's name/face cards to reflect the actual attendance at that moment.

Upon inspection of the facility fire safety records, it was determined that the required testing of the fire detection system occurred on the following dates, which exceeded the maximum of 30 days between required testing: 12.2.22-1.20.23; 5.17.23-6.20.23. Also, there was no valid receipt of purchase submitted for the fire detection system.
