During an Unannounced Inspection on 1/28/2025, 13 children were present in the Pre-K Counts 1 classroom. Staff #1 and Staff #2 were each asked to name and point out the children that were present in their assigned groups at that time. Staff #1 and Staff #2 each identified 6 children. Staff #1 and Staff #2 were not able to name or point out the one child in the classroom that had not been identified when they were asked again.

The policy must also include a plan for each staff to be assigned the responsibility for supervision of specific children, including the requirement that the staff person know the names and whereabouts of the children in his/her assigned group and be physically present with the children in his/her group on the facility premises and on facility excursions off the facility premises. The facility must create and implement specific procedures using a tangible supervision system, e.g. supervision cards, supervision lists, or another approved method, to ensure appropriate supervision at all times. The legal entity will submit policy/procedure to the Northeast Regional Office for approval. Once approved, all existing staff and new hires must be trained on the approved policies and procedures, and it must be documented at the facility.
