During attempts to verify the training and further questioning of Staff person #3, Staff person #3 disclosed they were aware that the training they submitted documentation for did not meet the regulatory requirements for fire safety training and only included training on fire extinguishers. Staff person #3 submitted a subsequent certificate for proof of fire safety training. Because the dates did not correspond to when Staff person #3 claimed the staff had taken training, the Cert. Rep. asked about the discrepancy. The LE stated that she had taken a certificate she found online and created her own certificate using the information from the prior document. The LE then submitted the first document again, along with a business card stapled to it. The name of the instructor on the business card did not match the instructor's name provided on the certificate, as well as including a different training date than the original document.
