During a renewal inspection conducted on 05/26/26, the Certification Representative reviewed Staff's #1 content of records and discovered Staff's #1 content of records attained the wrong FBI Clearance.

All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. A record of a person with a positive tuberculin skin test shall include the results of a chest x-ray and evaluation for chemoprophylaxis.

At time of inspection, staff person #1 ,3 and 4 did not have required pediatric first-aid and CPR training documented in their records.

During time of inspection , there was a play table with chipped paint and a table top on the Lego table with rough edges in room #103 which could be a hazard to children.
