This is self reportted. Per the report: Staff #1 told Child #13 times to go wash her hands. When child #1did not listen, staff #1 grabbed hcild #1 right upper arm and pulled them up directing them towards the sink. The directors were notified after the incident occurred. The directors watched camera footage. Staff #1 was asked to leave the building.

Exemption from immunization for religious belief or strong personal objection equated to a religious belief shall be documented by a written, signed and dated statement from the child's parent or guardian. At time of inspection, Child # 1 did not have updated influenza immunization documented in her record and/or a signed immunization form completed by the parent documented in the child's record.

At time of inspection, staff person #1 named in complaint was out on administrative leave pending completion of investigation. Director stated staff person #1 stated that the only physical contact she had with child's ear was when she was putting an earring back in her ear that came out. The squeeze of the ear to put the earring back in the child's ear may have been perceived by the child as too harsh and/or a form of punishment.
