The edges of a carpet in Classroom 1 were curling creating a tripping hazard. In the gross motor space, multiple photographs were hung on the wall, at the level of children, with nails that protruded approximately 3/4-1 inch from the wall. In the outdoor play space, 3 bricks, which had sharp corners, were by the porch area where children have access. A large branch (approximately 4-5 feet long) from a fallen tree was coming through the opening of the split rail fence into the upper play space which could cause injury to a child playing on the equipment. Next to the Armadillo Mine in the outdoor play space, a tree which had sharp branches and twigs extending from the branches were at child level and could cause injury to children playing in the area. At the xylophone in the outdoor play space, sharp branches were on the ground in an upward position and could cause injury to a child.

A sensory board on the railing of the porch in the play space contained a piece of a metal or aluminum gutter and a metal tube that had sharp, rough edges. The right-side and back wooden walls of the Armadillo Mine in the outdoor play space contained sharp rough edges in an area accessible to the children and could cause splinters.
