The back corner of a plastic cart, which was attached to a play tractor in Room 11, was broken presenting a hazard to the children due to exposed rough, and sharp edges. In Room 13, a red metal mesh basket was found with fraying along the top rim causing a sharp piece of metal to protrude upward and created a potential puncture hazard to the children.

Room 104 contained 3 12-ounce bottles of "Hand 2 Mind Iron Fillings." When measured, the radiators in the following areas had a temperature of at least 130 degrees Fahrenheit and were accessible to children: Room 11, Room 12, Room 13, Room 103, Room 104. Room 105, Room 106, Room 107, Room 108, Room 109, the 2 restrooms by Room 107, and in the hallway of Rooms 11, 12 and 13. In the Multipurpose Room, the hot water pipe which runs along the bottom of the entire wall and part of another, when measured, had a temperature of more than 140 degrees Fahrenheit. The hot water pipe in the corner of the girl's restroom downstairs, was not covered in an area that was accessible to the children and when measured, had a temperature of approximately 161 degrees Fahrenheit.
