Staff 1 was terminated. On 11.17.21 a supervision and ratio inspection was conducted. Staff in the brown bear room and blue horse room did not have sight of all children in their groups. Although the bathroom is located in the same classroom that the children are in, children go into the bathroom without staff's knowledge causing staff to not know the child's where about.
