The floors and table in the kitchen that the children eat in are not clean and in good repair. There is old, hardened food on the table the children eat on. There is an unplugged cord from the facility cameras hanging on a nail on the wall of the front childcare room used by the children, accessible to them. The outdoor play space has empty glass beer bottles accessible to the children in a box on the ground. The children's toys are not clean and in good repair on the outdoor play space and they are not free from visible hazards.

There is also a glass in a car seat on the ground in the back yard accessible to the children. There is a dogs wired leash wrapped and embedded around the tree in the back yard that is rusty and accessible to the children. There are also numerous empty beer cans in the backyard used by the children.

There is an extension plugged in the childcare room above the changing table, stretched across the kitchen floor and going out the backdoor. The kitchen refrigerator is filled with uncovered rotten food and food is spilled all inside it. There is a ripped couch in the kitchen with fruit flies flying all over it with dirty food dishes with old food on the back of the couch. The kitchen sink and stove are covered with used dirty dishes that have not been cleaned in days. There are used Q-tips on the ground in the kitchen near the base of the stove. It appears to be food and or mud smeared across the floor of the kitchen the operators are walking on in the kitchen. There are empty beer cans and bottles in the kitchen and outside. There is a pack of cigarettes on the kitchen table in the kitchen accessible to the children.
