Staff 1 had the pediatric first aid/cpr training expire in January of 2025 and was not renewed until 3/1/2025. As a result, staff 1 did not have an updated pediatric first aid/cpr certificate from 1/2025 to 3/2025.

During renewal inspection conducted 1/4/2024, child #1 was observed to be sleeping in an infant swing. Certification representative advised that infants cannot sleep in swings. At this time, staff person #1 took child #2 out of a pack-n-play and placed her on a cot. Staff person #1 then took child #1 out of the swing and placed him on his belly in the pack-n-play that was previously occupied by child #2. Child #1 is an infant (see LIS code sheet for date of birth) and there is no medical documentation on file requiring that child sleeps in this position. Certification representative advised that infants cannot be placed on their bellies, and staff person #1 then placed the infant on his back in the pack-n-play.
