Child # 1 did not have a fee agreement or emergency contact form completed and on file the day of their admission on 3/10/26. On 3/11/26 for the fee agreement was signed and on file and 3/12/26 the emergency contact form was completed and on file for child # 1.

3290.95(a)/3290.95(b) · Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.

Operator has 10-year lithium-ion fire alarms that are wireless and interconnected, but they are not tested in an approved testing laboratory required by the state.
