On 12/18/23, at approximately 7:45 am, it was observed that there was a lapse in supervision for an unknown amount of time. Upon entering the lobby/gym area it was observed that staff person 1 was across the lobby in the play area with 3 school age children. Upon entering the gym, it was observed that 5 school age children were alone in the room. One child was playing with a ball and the other 4 were climbing on a stack of chairs. One child returned to the gym after approximately 1 minute. Staff person 1 and the 2 remaining children returned to the gym after approximately 2 minutes.
