    CCL was unable to observe any Transportation logs during the visit, the provider Dannielle reported she had forgotten the transportation logs on her dresser at home this morning, a noncompliance was documented due to the provider not having the requested forms.

    CCL Campbell Payne observed the provider Rostte Stanley on the 4/27/26 partial visit leaving to transport with 2 children in the vehicle, one 9-year-old and one 4-year-old. Both children did not use or have any child passenger safety restraints.
