Assistant caregiver's PDL 1 expired 6/28/25. Assistant applied for renewal 7/15/25 but did not meet qualifications.
The living room does not allow space for the children to play due to furniture, a treadmill, two end tables, and an approximate 4 ft pile of donations, which included clothes, toys, and unassembled shelving. The dining area which is required for licensed space dining room table is covered with laundry.
Primary Caregiver's PDL expired on 12/5/2024 due to lack of professional development hours.
6 hours of 20 hours required of professional development is completed for this PDL renewal.
Caregivers' attendance records did not indicate the hours worked on the documentation.

Upon arrival there were 4 infants and only 3 playpens set up. Upon review of licensing one play pen mattress pad board was found to be broken. Meaning that only 2 playpens were available for use on this date.

Outside there was a climber that was not in use but accessible with broken PVC pipes. There was two tricycles with broken seat backs, one car with a broken door, play lawn mower with broken handle, and a slide with 4 corners that were broken. Edges on the equipment were sharp or a pinch hazard.

One year old child was asleep on the couch with no sheet on the surface. The only available play pen was being used by a one year old child. There was one infant in care on this date and no available sleep equipment for him. The playpen had three holes in the in the netting that need repair or replacement.
