Ongoing compliance with the requirements related to CHILDREN’S RECORDS: A home will maintain a complete record for each child, including drop-ins, completed before the child is admitted and kept at the home for 12 months after the child’s last day of attendance. Records will contain at minimum: Personal Information:, name of the child, date of birth, gender, home address, mailing address and telephone number, names of the parents or guardians, the parents or guardians current places of employment, addresses, pager, cellular and work telephone numbers, a list of people authorized to pick up the child and an authorization form signed by parent or guardian; identification of person authorized by the parent or guardian to pick up the child will be verified at pick up, date the child first attended the home and the date of the child’s last day at the home, a copy of the child’s up-to-date immunization record or a public health division approved exemption from the requirement. A grace period of a maximum of 30 days will be granted for children in foster care, homeless children and youth, or at-risk children and youth as determined by the department, a record of any accidents, injuries or illnesses that require first aid or medical attention and any observations of recent bruises, bites or potential signs of abuse or neglect, both of which will be reported to a parent or guardian, written authorization from the child’s parent or guardian to remove a child from the premises to participate in off-site activities; authorization will contain activity destination, date and time of activity and expected return time from activity, written authorization from the child’s parent or guardian for the educator to apply sunscreen, insect repellent and, if applicable, diaper cream to the child, a record of the time the child arrived and left the home and dates of attendance initialed by a parent, guardian, or person authorized to pick up the child, an enrollment agreement will be signed by a parent or guardian with an outline of the services and the costs being provided by the home, a signed acknowledgement that the parent or guardian has read and understands the family handbook. Any deficiencies will be corrected immediately, and findings will be documented and reviewed internally for continuous improvement purposes. I will check through all files to make sure all children's documentation of enrollment are marked, signed or initialed. In the event of any documents needing correction, I will take immediate action speaking to parents and have all documents marked and signed appropriately. Preventive Action I Rosa Barraza will check and monitor all files and documents of all new enrollments within a month after first day of attendance. To make sure all documents are filled and signed accurately. Compliance Monitoring Plan I Rosa Barraza will put this plan into action starting 4/28/26. The licensee does not have on file the following: the current child care regulations, the current list of notifiable diseases and communicable diseases published by the office of epidemiology of the New Mexico department of health, the latest licensing inspection report, the guidance policy, an up to date disaster preparedness plan approved annually by the licensing authority, a plan for care during a public health emergency. Provider has all documents on file, will post in visible area for parents to observe. Will check weekly to make sure all documents are posted and in good condition.
