8.9.4.25 · D KITCHENS
The refrigerator in the kitchen does not maintain a temperature of 41 degrees (Fahrenheit) or below. One side of the refrigerator has not been working since December. Only one side is working at the moment. Immediate Action: Upper management knows about the issue and is working on it. Director states when they came in to try and fix it the first time, the part they used to replace it did not work and had to order another part. The part should be in any day now to attempt to have it fixed again. Preventative Action: They have another refrigerator that works that they use, but it is not in the kitchen. When the part gets replaced, they will keep an eye on it to ensure that it works correctly. 8.9.4.23 · A PERSONNEL AND STAFFING REQUIREMENTS
A child was left unattended when the child opened the door to a hallway that has restrooms separating two classrooms and went to use the restroom. The educators were not aware that the child had left the classroom until the child was found by a federal reviewer. Immediate Action: Children's Services Manager reported the incident right away and when the incident occurred. The center also immediately was proactive and bought bells to place on the doors in the classrooms to ensure that they always know when the doors are being open. Preventative Action: Along with the bells being placed on the doors the center will ensure face to names will be done in the classrooms for the children. Ongoing Action: The center/Director will ensure that this is being done during the day to ensure that all children are attended for.

8.9.4.21 · B CAPACITY OF CENTERS
It was observed that children were cared for in an unlicensed area. Center started using a classroom in the elementary next door without approval from Licensing. PMS Management will need to get with Principal at school to come up with schedules for playground time and bathroom times as they will be sharing the spaces with the elementary's Pre-K program. Will not be approved to use classroom until then.

The center failed to post classroom capacities, and ratios and group sizes in an area of the room that is easily visible to parents, staff and visitors. From the review of staff records, it was determined that 8 out of 8 staff having direct contact with the children, does/do not have a complete file as required in 8.9.4.22F.
