From the review of staff records, it was determined that 1 out of 1 staff having direct contact with the children, does/do not have a complete up-to-date file in ECECD’s professional development information system (PDIS) as required in 8.9.4.22.F.1. See Staff Records 8.9.4.22 form for staff with an incomplete file. I will make sure to print the appropriate documents immediately and place in personnel file and perform periodic audits to ensure personnel files are complete. The trashcans in the classroom and the bathroom did not have tight fitting lids allowing odors from the diapers to contaminate the air in the room. 1.
