8.9.4.23 · A PERSONNEL AND STAFFING REQUIREMENTS
A child in the Toddler Room was left unattended when the Toddler Room went outside to the playground. The child was left alone for around 5 minutes before another staff member found the child sitting alone at the table. Immediate Action: Starting on 8/18/25, Director or member of management will supervise transitions to/from playground to provide on the spot coaching and to perform a sweep to ensure that all children are accounted for. Preventative Action: Starting on 8/18/25 Management will conduct daily random face to name checks to ensure compliance. All educators will be retrained on active supervision on face to name procedures. Logs of the retraining will be kept and reviewed by the District Manager. Also, only one classroom will be allowed on the playground at a time. Ongoing Monitoring: The District Manager will review face to names during all school visits, provide feedback and training to the Director and Management Team.

Educators in the 2 year old classroom are not actively engaged with children as seen on camera footage. There were children playing and laying in the shelves and another child standing on a chair and jumping off and the educator was not paying attention to the children. On 2/19/ 25 the educator in the 2 year old classroom was let go. 2/25/25 the Director is going into the classroom to coach if the educators need help and coming up with ways to keep the children actively engaged. Ongoing the Director will be watching camera footage to ensure that all the educators and children are actively engaged.
