The center failed to make a report to the licensing authority within 24 hours after the incident occurred regarding a lost or missing child. A child was left unattended in the class for approximately 11 minutes. Center did not report within 24 hours as required. Center did report later in the week. IMMEDIATE: Director will report asap to upper management. Oversight supervisor is responsible for reporting and will call in all incidents immediately as required. 8.9.4.24 · B NAPS OR REST PERIOD
A blanket/cloth was over the head or face of a child 12 months or younger when the child was laid down to sleep or when sleeping. This was in the 1 1/2 year old class. Two children had their faces covered completely TA was given regarding the children not having their faces covered. IMMEDIATE: Educator uncovered the children's faces PREVENTATIVE/MONITORING: Assistant Director stated this will be discussed with staff along with regular checking to ensure no faces are covered at any time 2 Year Old Classroom 8.9.4.24.B.9.:Illumination equivalent to that cast by a soft night light shall be operational in areas that are occupied by children who are napping or sleeping. Illumination must be enough to see the entire room, clearly observe sleeping children and allow for quiet activities for non-sleeping children. Finding Illumination was insufficient to observe the whole room. Corrective Action Plan TA was given to educators in the class about the lighting requirements. IMMEDIATE: This was corrected as the educators opened curtains for one of the windows PREVENTATIVE/MONITORING: Assistant Director stated they will ensure that the classes meet the lighting during nap and use the bathroom light if needed. This will be monitored with regular check ins. Preschool Classroom 8.9.4.24.B.3.:Cribs, cots or mats shall be spaced at least 30 inches apart to permit easy access by adults to each child. If the room used for sleeping cannot accommodate 30 inches of spacing between children, educators shall space children as far as possible from one another. There must be enough room to permit easy access to all children without moving cribs, cots or mats. Cribs which have sneeze guards installed may be placed end-to-end as long as they remain easily accessible. 8.9.4.23 · A PERSONNEL AND STAFFING REQUIREMENTS
The child(ren) in the 2 yr. old Early 2's class and the Older 2's class and Also the Early 3's and Younger 3's class room(s) was/were left unattended when Educators were not attending to all children on the playground. Educators for the classes were lining up to come in while one child was alone by the gate on the north side of the of the playground. In the 3's classes, while doing a head count, licensor did not come up with same head count as educator provided. Educator stated it was probably a child who likes to hide alone in the playground equipment. As she went looking for him, he was located on the other side of the playground unattended riding a bike. Director will discuss line of sight with all educators. Director will discuss plan with all educators about how to keep all of the children together when they are doing activities on the playground. Director and Assistant will do checks regularly to ensure all children are attended at all times whether inside or outside.

8.9.4.23 · B STAFF QUALIFICATIONS AND TRAINING
From the review of staff records, it was determined that 6 out of 17 staff working more than 20 hours a week, has/have no documentation of at least 24 hours of qualified annual training, See Staff Records 8.9.4.23 form for staff with missing documentation of training. Director has educators scheduled for trainings that are upcoming and will monitor the training log properly to ensure they are completing all required hours. A prohibited guidance practice was used in the 4 yr. old class room(s) as evidenced by the use of yelling. The educator was overheard telling a child to "Change your clothes, NOW." Educator stated child was being told to change out of her swimsuit for not listening, but that she would allow her to change back into it before they went outside. The educator's tone was not appropriate. Director will speak with Educator regarding tones and proper redirection. Director is also going to assign a training to the Educator. The use of physical or mechanical restraints is prohibited unless due to documented emergencies or medically documented necessity. Finding The center staff used physical or mechanical restraints on a child by placing child in highchair for 15 minutes.
