The child(ren) in the School Age class classroom was/were left unattended when a child was not being supervised while outside. The children were playing a group activity while a child was away from the group and was pulling on a stick on a tree that eventually came off with full force, poking the child in the eye requiring medical attention. . Director immediately wrote up the educator. Director also conducted an onboarding orientation and specifically covered supervision at all times and always having the children in line of sight. Director provided documentation of the write up and the meeting as well.
