The equipment in the playground area are not safe in that the slide was too close to the sidewalk. The slide was 4 feet away from the sidewalk, however a child was able to pull a child off the slide towards the sidewalk causing her to hit her head. IMMEDIATE: Director had the vendor remove the structure from the area it was in and move it back 4 feet further from the sidewalk to prevent any further incidents. I SPOKE TO TEACHER AND I WILL REMIND TEACHER OF STATE REGS AND LET MY DIRECTOR know today what happen we will together call teacher in and have a write up for teacher to sign Children were left unattended due to using the building that he did not have access to use. When compliance officer was walking out of the building to finish the survey, and the educator asked to leave the door open and asked children who needed to go to the restroom. Compliance officer went back to the main building. Two other educators called the assistant director to go outside when they noticed that the educator did eventually let the door close behind him. Compliance officer was already inside when assistant director was called.
