8.9.4.23 · A PERSONNEL AND STAFFING REQUIREMENTS
The child(ren) in the 3 yr. old class room(s) was/were left unattended when the child was allowed to leave the class and go to the restroom unattended. 8.9.4.23 · B STAFF QUALIFICATIONS AND TRAINING
Educators did not complete the following training within 3-months: first aid and cardiopulmonary resuscitation (CPR) certification, CPR Health and Safety Training mandatory trainings Director stated some of the educators have completed the 1st Aid CPR and they are awaiting for certificates to get them into the files. Director stated educators are also working on completing the Health and Safety trainings. Director has training logs and will ensure that they are used accordingly moving forward. 8.9.4.23.B.2.c.:New staff members working directly with children regardless of the number of hours employed per week will complete the following, or a three-credit early care and education course or an equivalent approved by the department prior to or within twelve months of employment or the effective date of these regulation amendments. 8.9.4.29 · H SAFETY COMPLIANCE
The center failed to conduct an emergency preparedness practice drills for at least once a quarter. Director showed licensing her weekly to do log which includes a spot for fire drills and emergency drills. This will be used for scheduling the drills so that they are not missed. TA was also given regarding the drills not being conducted on the same day.

The licensee did not obtain background checks on all staff members, educators, volunteers, and prospective staff as per the requirements outlined in the department's most current version of the Background Check and Employment History Verification provisions. Employee in question works for The school district, but also was on our payroll as an after hour maintenance worker. Because we had a hire packet for him but without the background/fingerprinting, I missed it. As soon as we were made aware, we did not have this employee back, and will not have him back until he is cleared. He has an appointment tomorrow. Pillows are used in cribs. Evacuation cribs were used as storage for educators belongings, diaper bags, and shoes. The director will discuss with the educators that the evacuation cribs need to be clean and clear at all times. Pillows in the cribs will also be discussed. I/T Classroom #2 8.9.4.24.C.2.:Cribs will meet federal standards and be kept in good repair. The center will not use plastic bags or lightweight plastic sheeting to cover a mattress and will not use pillows in cribs. Stacking cribs is prohibited. Cribs will not be used for storage. Animals and pets will not be allowed in cribs or on sleeping materials. Finding Pillows are used in cribs. Evacuation cribs had diaper bags, shoes, and personal items for educators. Corrective Action Plan The director will discuss with the educators that evacuation cribs are needing to be clean and clear at all times.
