The center failed to display in a prominent place that is readily visible to parents, staff and visitors the most recent fire inspection report. Immediate - Director contacted fire marshal to obtain a copy of current fire inspection report, Fire inspection occurred in April 2025. Preventative - Effective 12/19/2025 Director will get a copy of the current fire permit after each fire inspection. if inspection report is not available, director will set reminders to follow up with fire marshal timely. The premises are not in good repair as evidenced by in early Prek and Pre 2. Immediate - Director will have a work order submitted to have light bulbs replaced on or by 1/18/2026. Preventative - Eff 12/19/2025, Classroom inspection log will be broken into subsections that includes bulbs for educators to check weekly. If damages are found educators will submit a request to director have a work order completed. Monitoring - Director will conduct routine reviews of classroom inspection logs to ensure each classroom are filling them.

The center failed to notify parents or guardians in writing of an incident regarding the child was grabbed by an educator aggressively. . The director will check daily for all incident reports being singed and given to parents. The director has a clipboard for all incident reports going out and she signs the incident reports, also the educator who close's gives incident reports to parents for signature. The director will put incident reports in children's files after reviewed and signed. 8.9.4.21.C.1.:The licensee will report to the appropriate authorities the following incidents. After making a report to the appropriate authorities, the licensee shall notify the licensing authority of the incident giving rise to its report as soon as possible but no later than 24 hours after the incident occurred. Educators in the Preschool class room(s) failed to model appropriate social behaviors, interactions and empathy. They failed to respond to children that were angry in a caring and sensitive manner when grabbing the child by the arm aggressively. . The Director did terminate the educator. The Director will have a training at the next staff meeting, which is on May 23, 2025, on modeling appropriate behavior.
