Room(s): I/T Classroom #1 : Not Accepted 06/08/2026 Immediate Action: On 5/22/26, Jacqueline Padilla, EHS Teacher, added the capacity for each interest area on the area labels in her classroom. Preventive Actions: On 5/23/26, the Center Supervisor, Cassia Cook, monitored each classroom to ensure that their intrest area signs are posted with the capacity for each area. Compliance Monitoring Plan: At the beginning of each school year, the Center Supervisor, Cassia Cook, will monitor each classroom ensuring that each classroom have interest area signs posted with the capacity. Guidance being used by staff in the 2 yr. old room(s) is not appropriate as evidenced by an educator yelling at a child. Immediate action: Educator submitted her resignation effective 6/30/2025, prior to her being terminated per director. Preventative action: • Ongoing Training and Procedure Review • A staff meeting was conducted with all Early Head Start staff on June 26, 2025, to review Procedure 7200.017 – Direct Guidance and Discipline Procedure. • Ongoing training will continue to reinforce appropriate practices. • Staff Supervision and Support • Supervisors will proactively discuss any frustrations during individual check-ins with staff, providing a supportive environment to help prevent future incidents. • Break Time Enforcement- Staff will be reminded and encouraged to take their full 15-minute breaks to promote mental and emotional well-being. • Classroom Observations- Regular classroom observations will be conducted to ensure that: Teachers are providing developmentally appropriate guidance. Instruction aligns with age-appropriate best practices. Peer Support- Staff will be reminded to support co-teachers who appear to be frustrated, and encouraged to step away briefly when needed to self-regulate. Compliance monitoring: Starting Immediately, • Staff Check-Ins- Maintain regular supervisory check-ins with staff to address concerns and provide ongoing support. •Guidance and Discipline and Staff Wellness will be standing agenda items at all staff meetings to ensure continued focus and accountability. • All staff will receive a refresher training on guidance and discipline practices during August 2025 Pre-Service. • Access to Support Resources - Ensure all staff have access to: Employee Assistance Program (EAP); Mental Health Consultant; Additional wellness and support resources
