8.9.4.23 · A PERSONNEL AND STAFFING REQUIREMENTS
The children in the school age classroom was/were left unattended when the director left 5 children in the classroom to go open the door for another educator and Focus Verifier. They were left alone for less than a minute. 6/11/24 the provider called to let us know of the incident that occurred and let us know that the issue was corrected immediately. 6/11/24 provider and verifier discussed what happened and discussed what needs to be done so it does not happen again. 6/12/24 going forward the educators will ensure that no child is left unattended. the director and educators will ensure that the children will always be in their line of sight.

8.9.4.29 · H SAFETY COMPLIANCE
The center’s fire extinguishers are not inspected yearly. On 6/10/24 Director will call the person who inspects the fire extinguishers and setup a time for him to come out. On 6/10/24 Director will put a calendar reminder to ensure that the fire extinguishers get inspected yearly. Effective 7/1/24 Director will perform quarterly inspections of the fire extinguishers just to ensure the tags are up to date.
