The energy absorbent material under the slide in the small playground had been strewn about so that only the tarp was visible in the fall zone. The center failed to notify parents or guardians in writing of an incident regarding: on 3/4/25 mother received a text message stating that her child had a disagreement with another child during outside play time, and her child was punched in the stomach. Mother was never given an incident report. IMMEDIATE: Director conducted a staff meeting and discussed the requirement of incident reporting with all staff PREVENTATIVE/MONITORING: Director will do regular checks ins with educators to verify that any incidents have been documented for parents to sign.
