8.9.4.31 · C Incident Reporting Requirements
The home failed to notify parents or guardians in writing of an incident regarding the parent's child falling near the sandbox. Immediate: If a child trips, falls, or experiences any accident while in my care, I will complete an incident report immediately. The report will include the time, location, description of the incident, and the care that was provided. Parents will be notified right away, and a copy of the report will be given to them the same day. I will have the parents sign the original incident report as their acknowledgement of the incident and keep on file. If the injury requires medical care beyond basic first aid, I will also notify the licensing authority within the required 24-hour timeframe. Preventative: To reduce the likelihood of future accidents, I will continue to conduct regular safety checks of both indoor and outdoor environments, ensuring play areas remain free of hazards and children will continue to be closely supervised. Monitoring: I will maintain all incident reports in each child’s individual file to ensure accurate record-keeping and compliance with licensing requirements. These files will remain organized, up to date, and available for review by licensing at any time. Administrative Requirements
