A blanket/cloth was over the head or face of a child 12 months or younger when the child was laid down to sleep or when sleeping. **an infant was swaddled in the crib with a blanket. The child(ren) in the PreK Class room(s) was/were left unattended when a child was left alone in the bathroom while the rest of the class went outside to the playground. A parent that was walking into the building heard the child cry and notified a teacher. The director will create a procedure which will include a thorough briefing before shift changes, including discussing the day’s agenda, the number of children present, and any pertinent information for families. Educators completing the transition will sign the attendance sheet confirming their understanding before the shift changes. The director will train the educators on the new procedure to ensure they understand the steps they must complete to ensure no children are left unattended.
