Immediate-Form was taken out and put by the door so that parent can sign once she comes to the home today. Preventative- Will ensure to have pet notification formed annually. Preventative: Provider will continuously inspect play equipment and remove hazardous equipment and maintain the play area in a safe condition to ensure the ongoing safety of children.
