The center failed to immediately report any incidents involving suspected child abuse and neglect to children's protective services and local law enforcement. Room(s): Admin/Licensure : Accepted 05/19/2026 In response to the violation on 05/07/2026 by the New Mexico ECECD, immediate action was taken on 05/19/2026 by the Executive Director to inform teachers involved in the survey report that it was determined by licensing compliance that the school/teachers should have taken action sooner to report suspected neglect/abuse to authorities. Discussion included better communication of the "full picture" between teachers and administrators. Administrators were singularly unaware of the number, variety, and degree of indicators observed by the teachers. Administrators have been advised to ask "what else" when a teacher brings a particular incident to their attention; teachers have been advised to say "and also..." when they bring a concern up. Teachers were also advised that we could call the local CYFD office to ask whether or not an event or incident was deemed reportable. "Suspected abuse/neglect" has been added to the recurring monthly staff meeting agenda template, so that it is addressed at a minimum, monthly. To enhance the currently compliant operations, on 05/19/2026 the Prek Administrator will At a staff training day on April 3, 2026, we had a trainer from CYFD come and give a session on abuse/neglect reporting. Many issues were discussed at this meeting. At the time we determined we needed to do addition/ongoing training in this area. The PreK Administrator will reach out again to CYFD to complete additional in-person staff training. A date is yet to be determined, and will either be part of a regular staff meeting, or on a teachers work/training day. We will request that we have a minimum of 3, 1-hour training sessions over the next several months, with preference given to Aug., Oct., and Feb, with a completion date of 02/26/2027. Effective 05/19/2026 the Executive Director will perform monthly checks of bring awareness/reminders/questions to light, through 04/30/2027 to maintain ongoing compliance with “Questions/Concerns/Discussion – Child abuse/neglect indicators” has been added to our monthly agenda, every month. After the 3 trainings referenced in Preventative Actions, the Assistant Director will schedule an annual training with CYFD on child abuse/neglect prevention, preferably in the month of Feb. Any deficiencies will be corrected immediately, and findings will be documented and reviewed internally for continuous improvement purposes. The center combines age groups of children. The required group size for the Age in group: two through four years - Maximum group size: 24, Age in group: three through five years - Maximum group size: 28. The observed group size was two through five years. There is no evidence of inadequate staff-to-child ratio, however Center has been combining classrooms ranging from 2-year-olds through 5-years-olds during their end of day "aftercare" session outside on the playground. Room(s): Preschool Classroom : Not Accepted 05/18/2026 In response to the violation on 05/07/2026 by the New Mexico ECECD, immediate action was taken on 05/15/2026 by the Executive Director to Immediate Actions: The Executive Director has updated our staff policy and handbook to reflect the combined age group requirements (ages and group size), and we are collecting staff signatures that they are knowledgeable about the updates and will comply with age grouping requirements and maximum group size. Staff are informed (via the statement) that failure to comply with requirements is grounds for disciplinary action. The Assistant Director has adapted the playground schedule to avoid inappropriate age mixing and group size. To enhance the currently compliant operations, on 05/15/2026 the Executive Director will The Executive Director has met with two independent contractors for estimates on fencing the playground into 3 sections to help separate groups of children during outdoor play. From the review of staff records, it was determined that 15 out of 31 staff does/do not have documentation of the 45-hour entry level course or an approved equivalent prior to or within six months of employment. Management is working to find all certificates for employees who have completed this. Management has changed in the center and not all certificates have been located.
