The changing table has a heavy accumulation of dirt/sand underneath changing mat. Immediate: Educators corrected on site by cleaning the dirt/sand from underneath the changing mat. Preventative: Compliance officer gave T/A and educators stated that they will clean underneath mats more thoroughly when changing. Monitoring: Director will do random checks in classrooms to check that changing tables are clean and free of dirt.
