From the review of staff records, it was determined that 3 out of 9 staff working more than 20 hours a week, has/have no documentation of at least 24 hours of qualified annual training, See Staff Records 8.9.4.23 form for staff with missing documentation of training. Immediate Action: Director will have educators print out their trainings today 3/11 and put them in the files. Preventative Action: During monthly meetings the director and educators will print out any trainings that they have done for the month and will put them in their files. Ongoing Action: Director will complete quarterly audits of the employee files to ensure that everything is up to date.

The water bottles brought from home for children in both of the classrooms are not labeled. Immediate Action: On 03/27/25 Director was made aware that some bottles are not labeled. Director had educator label bottles. This was corrected onsite. Preventative Action: Starting 03/28/25 Director will add bottle labeling as part of their morning duties checklist. Ongoing Action: Director will double check that bottles are labeled daily.

The center does not have hot and cold running water pressure in bathrooms. On 10/3/24 director was notified that there is no hot water pressure in the children's restroom in one of the classrooms. 10/3/24 director will call and have someone to come and put a gauge in the sink in the children's restroom. Starting 10/8/24 educator's will do daily temperature checks on the sinks in the children's restrooms when they arrive to work.

Educators in the Preschool classroom(s) failed to respond to children’s questions and acknowledge their comments, concerns, emotions and feelings when the child appeared to be hurt and the educator did not render proper first aid to Childs scratch. Staff meeting will in be completed and a updated incident report policy will be acknowledged by all educators.
