On 10/10/2024 the second care giver will log her hours on a sheet of paper. 2. As of 10/10/2024 the second care giver will place the log sheet next to the sign in and out sheet that is filled for the children each day. This will help to remind her to log her hours worked. 3. On 10/10/2024, the second care giver will draft a child abuse statement and will sign it yearly. The primary caregiver will also sign this annually. 2. As of 10/10/2024 the second caregiver will put the signed annual statement in the binder next to the training log. This will help remind her that it needs to be done yearly. 3. As of 10/10/2024, the second caregiver will go through the binder every month to make sure it is up to date and all required documents are included and up to date. This will be an ongoing effort. 8.9.4.34 · J Outdoor Play
The fall zone underneath the swings is not adequate as evidenced by the absence of energy absorbent material - the surface is bare soil. 1. As of 10/10/2024, the swing will be removed until they get the supplies to build up the energy absorbent material. 2. On or before 11/1/2024 the provider will ensure that the area is adequate for the intended use at the end of each day. 3. On or before 11/1/2024, the provider will make sure that the playground is safe and ready to use. The playground will be inspected each night to ensure that accident prone areas are safe. Finding Playground equipment is not safe as evidenced by the following: sharp or jagged bricks in toddler play area. Corrective Action Plan 1. On or before 10/23/2024, the provider will remove the blocks from the toddler play area. 2. 8.9.4.38 · A Housekeeping
The gate leading downstairs off of the back deck is not in good repair as evidenced by a missing latch. This is a concern due to children being able to open the gate and falling down a flight of stairs. Also, the sliding glass door leading from the kitchen to the back patio/deck is difficult to open. The track may need cleaned and the rollers may need replaced or adjusted and cleaned. 1. On or before 11/1/2024 the provider or secondary caregiver will repair or schedule repairs to the sliding glass door and the latch on the gate. 2. On or before 11/1/123024 the provider or secondary caregiver will spontaneously check the operating mechanisms on all doors and latches around the home to ensure they are working properly. 3. On or before 11/1/2024, the provider or secondary caregiver will be prompt about making repairs to doors and gates as soon as there are issues. This will be an ongoing effort. Areas of confinement for pets, such as cages and pens, and outdoor areas were not cleaned of excrement daily. There are also large holes in the play area being dug out by the dog. The holes will need to be filled with dirt to prevent possible injuries to walking or running children and adults. 1. On 10/10/2024, the dog excrement will be removed from the play area. 2. As of 10/10/2024 the secondary caregiver will check the playground for dog excrement before children are permitted on the playground. 3.
